How VCSE and Primary Care Network partnerships can tackle health inequalities

10GM are hosting an online lunch and learn session to explore how effective VCSE and Primary Care Network partnerships can tackle health inequalities in GM on 5 September, 12pm-1.45pm.

10gm a joint venture to support the local vcse sector in greater manchesterYou are invited to join 10GM to hear about and discuss learning from a project that has tested out how to improve relationships between Primary Care Networks (PCNs) and the Voluntary, Community and Social Enterprise Sector (VCSE) in Greater Manchester.

10GM has been working with partners from Population Health, Primary Care Networks and the wider VCFSE sector to find innovative ways of tackling health inequalities. This included establishing 5 Test and Learn Sites to explore how the VCSE sector and GPs can build lasting relationships that help them work together to tackle health inequalities and better meet the needs of local people.

At this event you will:

  • Hear directly from 5 test and learn sites from the following Primary Care Networks: Ardwick and Longsight (Manchester), Sale Central (Trafford), Victoria (Stockport), Tyldesley, Atherton, Boothstown and Astley (Wigan) and Eccles and Irlam (Salford) about their partnership work and the impact it has had
  • Gain insight into evaluation and learning from the project, including the enablers of effective relationships between PCNs and VCSE organisations
  • Understand how the CORE20PLUS5 model of inequalities can act as a framework to drive targeted action in health inequality improvement

You will gain:

  • An understanding of how long term, partnerships can be formed and sustained
  • A bank of good practice and practical examples of how collaborative working between VCSE organisations and PCNs on local population health priorities can tackle inequality and improve health and wellbeing of local communities
  • Greater clarity on the barriers and enablers, resources, and leadership for effective relationships between VCSE organisations and PCNs

The event is jointly hosted by 10GM, GM Population Health, the PCN/VCSE project partnership, and Matthew Baqueriza-Jackson Ltd and is part of a series looking at how we can create Fairer Health for All in Greater Manchester. Fairer Health for All is a system-wide commitment and framework for reducing health inequality and tackling inequalities across the wider, social determinants of health, as well as creating a greener, fairer, more prosperous city-region.
