The purpose of this survey is to gather the views of patients and the public regarding the distribution of health information leaflets via North West Ambulance Service’s Patient Transport Service (PTS). 
In taking part in this survey the participant is giving their consent for their responses to be used for the purpose outlined above. The identity of all participants will remain anonymous and taking part in the survey will have no detrimental effect on their use of Patient Transport Services in the future.
