UCLAN Research Project - Innovation in care pathway for people with long term conditions

The University of Central Lancashire and Zest Healthy Living Service are looking for people who are resident in Harpurhey and who are either living with or caring for someone with one or more long terms conditions to take part in the study.

About a quarter of people in Harpurhey are thought to be living with a long-term health condition. Long-term health conditions cannot be cured, but they can often be managed or controlled by some form of medical treatment or by some other form of intervention.

Examples of long terms conditions include:
Hypertension
Obesity
Asthma
Diabetes
Coronary heart disease
Cardiovascular disease
COPD
Chronic kidney disease
Hypothyroidism
Stroke/TIA
Cancer
Epilepsy
Dementia
Mental health, (including depression and anxiety)
Sickle cell
Plus others

The Public Health Department at Manchester City Council and the North Manchester Clinical Commissioning Group (CCG) have asked the University of Central Lancashire to work with Zest Healthy Living Service to undertake a study to look at how care for people living with long term conditions in Harpurhey could be improved. The study will look in particular at the role that social networks (that is, the people that we know and the relationships that we are a part of) can play in helping to improve people’s lives.
